Palliative Care Education Passport

The award winning passport was initially developed by Springhill Hospice and has been endorsed by Skills for Care, the Bolton Hospice education team are now able to deliver this at Bolton Hospice. The programme consists of 6 half-day modules aimed at all roles and disciplines to provide the knowledge and skills required to care for someone in the last years of their life.

Module 1 – Advance Care Planning/recognising advanced disease

Module 2 – The REAL Plan – foundation level communication skills

Module 3 -  Psychological and spiritual support for people and their families

Module 4 – Symptom management and assessment

Module 5 – Recognising dying and care at the end of life

Module 6 – Care after death and bereavement support
